The main porblem I think is when webmasters try clicking their own ads. Never do it. Instant ban from Adsense.

I think it's the advertisers - ie, Adwords customers - who are most concerned about getting ripped off. Most publishers are happy with the cash, even though they would make far more money with direct advertising contracts. Many Adsense publishers are literally giving away their advertising space.
